I have cretaed a simple form in Dreamweaver and uploaded it to mywebsite. how to I go about adding a button that will collect the information and send it to me via email?

Oh dear you will need more than a button to do all of that. You will need a server side script to send the information on to you. You might want to have a look at: PHP Tutorial

PHP Forms: PHP Forms
