Kinda off topic: What does "/" really mean?

Can anyone point me to the official meaning of the "/" (slash, stroke whatever) in a sentence?

Does it mean "And"
Does it mean "And/Or"
Does it mean "Or"

I do a lot of corporate graphics and this issue comes up quite frequently when translating engineers' sketches into meaningful infographics...
