Need help with technical term

I know what I want, but don't know how to ask for it. Here it is in a nut shell:

On my website, I want my visitors to able to provide some simple data about what they want on a one page form.
EX:
Number of Adults___________
Number of Children_________
Pets __Y___N___
Area of Visit _____Radio button with different areas of my state____
Desired Activities ____Radio buttons with different activites_____

Then once they fill it out and submit it, I would like the data provided on the form to generate a list of possible desinations tailored only to what they requested.
EX: if they want to go up north and Boat, then I only want lakes in that area to be a destination option.

I was searching the web for examples and did some searches for "database search" and "search engine," but everything I find on those keywords is like google or yahoo. I was also thinking that maybe I could create what I'm looking for through a shopping cart program, but unsure.

Honestly I just need to figure out how to word it to a programmer so that there is no confusion when I open it up for bidding.

Any help would be great...

These are all attributes for each "destination" option. So you can tell the programmer these are search fields, and when submitted, it brings up all all the database entries with these attributes.

Pretty much. That's a very simple task. Should only take someone a couple hours to do with php.
