damn I just wrote really long a step by step and aciddently deleted it!!!
well i'll write a short version
4 layers
1. rounded rectangle which have 2 strokes and outer glow
2. header which has a gradient
3. the diagonal lines with a low opacity (over the header)
4. the light green under the title which will just be a solid color with a gradient mask
there are many other ways to create that effect in but this is the way i would be following if i were to copy that rectangle.
