There's nothing to pay for the photoshop file. The $45.95 is for the labels.
Just right click on the link that says > "Download Template for Adobe PhotoShop" and do "save target as". I opened the file and used it in both Photoshop and Illustrator. (I got a better printing resut using it within Illustrator)
