If it's a flash ecard to be viewed online, you'd have a script which would send an email saying words to the effect of "dear granny smith, little johnny has sent you an ecard which you can view here <LINK>" otherwise it'd just attach the mail with the card itself. 101 ways to do it, none of it too taxing.
